Therapy After IOP — Support That Continues Where Your Program Left Off
Completing an Intensive Outpatient Program is a real accomplishment. It’s also one of the most vulnerable moments in a person’s mental health journey — the moment when the structure ends, and the real work of building a sustainable life begins. At Real Therapy, we specialize in what comes next.

Most outpatient therapy treats every client the same way. We don’t. Clients coming out of IOP or PHP have done significant work. They’ve built insight, practiced skills, and shown up even when it was hard. What they need now isn’t generic weekly therapy — it’s a clinician who understands where they’ve been, what they’ve built, and how to help them sustain it.
Our clinicians are trained social workers (LICSW, LCSW, MSW) with deep experience in continuity of care. We coordinate with referring programs whenever possible, and we take the handoff seriously — because that transition window is when people are most at risk of losing ground.

Your first session isn’t an intake form. It’s a conversation. We want to understand what you’ve been through, what’s worked, what hasn’t, and what you’re hoping to build from here. From there, we work together to create a plan that actually fits your life — not a template.
Sessions are held virtually, which means you can access care from wherever you are in Massachusetts without rearranging your schedule around a commute. For clients coming out of structured treatment, that flexibility often makes the difference between consistent attendance and gradual drop-off.

We understand what a good step-down referral requires. When you refer a client to Real Therapy, you’re sending them to a licensed, clinician-owned practice that takes the handoff seriously. We’re available for coordination calls, we welcome treatment summaries, and we’ll let you know when a client is safely engaged in care.
If you’d like to discuss a specific client situation or learn more about how we work, reach out directly. We’re always glad to talk through whether we’re the right fit before a referral is made.
